Substantive Change

Directions for Completing and Submitting a Substantive Change
1. Review ACEN Policy #14 Reporting Substantive Changes to determine the type of substantive change, the applicable review procedure, and the required submission timeframe.
2. Access the Substantive Change Guidelines on the ACEN Resource Center to locate the appropriate template for your substantive change type.
3. Download and complete all sections of the applicable template. For changes involving multiple program types, complete and submit a separate report for EACH program type except for Nurse Administrator Changes.
4. Substantive Change Reports must be submitted as searchable PDF files or Word documents. Do not submit the report as a scan of a physical document. Only appendices may be submitted as PDFs from scanned documents.
5. Submit the completed report through the ACEN Substantive Change Portal.
